The combination of heart and sport has been a topic for over a century. The benefits and risks that are involved in relation to sport have been debated and the ECG has been a constant focus of these discussions. We know with certainty that regular endurance sports activities lead to a physiological increase of the heart's size, accompanied by a regular, eccentric increase in size of all heart chambers, and to a thickening especially of the membranes of the heart chambers accompanied by improved cardiac functions. As a result, ECG alterations can be observed, which are due to this physiological adaptation. However, these alterations can be misinterpreted as pathological changes. During several consensus conferences, recommendations on the normal ECG of athletes and on clearly pathologic findings (cardiomyopathy, channelopathy) have been established.

Depuis plus d'un siècle, la combinaison du cœur et du sport fait l'objet de nombreuses discussions. Les risques et les avantages associés à la pratique d'un sport sont au cœur de ces discussions et l'ECG en demeure le point central. Nous savons avec certitude que les activités sportives d'endurance entraînent une augmentation de la taille du cœur, accompagnée d'une augmentation excentrique de la taille des cavités cardiaques, ainsi qu'un épaississement des membranes des cavités cardiaques, accompagné par une amélioration des fonctions cardiaques. Ainsi, des modifications de l'ECG résultant d'une telle adaptation physiologique peuvent être observées. Herz und Sport, das ist ein Thema seit über 100 Jahren. Immer wieder wurden Nutzen oder Gefährdung durch Sport diskutiert und das EKG stand und steht meist im Mittelpunkt der Diskussion. Gesichert ist, dass regelmäßiger Ausdauersport zu einer physiologischen Vergrößerung des Herzens führt mit gleichmäßiger, exzentrischer Größenzunahme aller Herzkammern, und Wandverdickung vornehmlich der Herzkammern bei verbesserter kardialer Funktion. Als Folge werden EKG-Veränderungen beobachtet, die Folge der physiologischen Anpassung darstellen. Diese können aber mit krankhaften Veränderungen fehlinterpretiert werden. Mehrere Konsensuskonferenzen haben hier Klarheit geschaffen mit Empfehlungen über das normale EKG bei Sportlern und über eindeutig pathologische Befunde (Kardiomyopathien, Jonenkanalerkrankungen).
