The undersigned, having acquired the information provided by the processing controller pursuant to art.13 of Law Decree 196/2003, is aware, in particular, that processing will concern the “sensitive” data indicated by art. 4 sub-section1 letter d), and art. 26 of Law Decree.196/2003, i.e., the data “suitable for determining racial and ethnic origin, religious, philosophical and other convictions, political opinions, membership of parties, unions, associations or organisations of a religious, philosophical, political or union nature, as well as data suitable for determining state of health and sex life”.

Le soussigné/la soussignée, après avoir pris connaissance des informations fournies par le responsable du traitement conformément à l’art. 13 du Décret Législatif 196/2003, est informé(e) du fait que le traitement concernera les données “sensibles” visées à l’art. 4 alinéa 1 lettre d), ainsi qu’à l’art. 26 du Décret Législatif 196/2003, à savoir les données “pouvant identifier l’origine raciale et ethnique, les convictions religieuses, philosophiques ou de tout autre genre, les opinions politiques, l’adhésion à des partis, syndicats, associations ou organisations à caractère religieux, philosophique, politique ou syndical, ainsi que les données pouvant identifier l’état de santé et la vie sexuelle”.

Il/La sottoscritto/a, acquisite le informazioni fornite dal titolare del trattamento ai sensi dell’art.13 del D.Lgs.196/2003, e consapevole, in particolare, che il trattamento riguarderà i dati “sensibili” di cui all’art.4 comma 1 lett. d), nonché art. 26 del D.Lgs.196/2003, vale a dire i dati “idonei a rilevare l’origine razziale d etnica, le convinzioni religiose, filosofiche o di altro genere, le opinioni politiche, l’adesione a partiti, sindacati, associazioni od organizzazioni a carattere religioso, filosofico, politico o sindacale, nonché i dati idonei a rilevare lo stato di salute e la vita sessuale”.
