Mae’r proffiliau, a gyhoeddwyd gan Arsyllfa Iechyd Cyhoeddus Cymru, wedi cael eu cynhyrchu i gefnogi datblygiad grwpiau bach o bractisau meddygon teulu sy’n gweithio’n gydweithredol, a elwir yn glystyrau o feddygon teulu. Mae’r proffiliau’n cynnwys dadansoddiad disgrifiadol o’r boblogaeth gofrestredig o ran lledaeniad daearyddol, strwythur oed/rhyw, amddifadedd, nodweddion gwledig/trefol a mynediad i ofal sylfaenol.

The profiles, published by the Public Health Wales Observatory, have been produced to support the development of small groups of GP practices working collaboratively, called GP clusters. The profiles contain descriptive analysis of the registered population in terms of its geographic spread, age/sex structure, deprivation, rural/urban characteristics and access to primary care. In addition, the profiles contain information on chronic disease for a number of conditions that have a high impact on services:
